Chronic Disease Management: Qualified professionals guide individuals to take simple, actionable steps that aid in improving quality of life and slow the progression of chronic disease. Chronic disease management can assist with symptoms caused by COPD, diabetes, congestive heart failure, and many other diagnoses.

Foot Care and Nail Care: Healthy feet are crucial for maintaining mobility and independence, which in turn improves overall wellbeing. Foot care exams and toenail clipping can help detect and prevent infections and sometimes even reduce pain. Good hygiene and self-care behaviors are emphasized and education about warning signs and proper nail care is provided.
- Health Promotion and Disease Maintenance Education: Have you been given a new medical diagnosis? Our nurses are here to equip you with the knowledge, education, tools, and support you need to improve your overall health outcomes.
- Immunizations – Can be provided in your home or at our office.
- Medication Management & Set Up: Foster County Public Health nursing staff will pick up your medications from the pharmacy, package them in medication packets as ordered, and then deliver them to your home. Education on medications is provided with all new medications and as needed. Vital signs including blood pressure, temperature, pulse, and oxygen saturation are taken quarterly and as needed.

Reproductive and Sexual Health Services
Reproductive Health Services addresses infertility, a wide range of risk behaviors and health conditions, as well as contraception. Services include:
- Physical exams for both men & women
- Birth control methods
- Breast, cervical, and testicular cancer screenings
- Diagnosis and treatment of sexually transmitted infections
- Pregnancy testing
- Counseling and education
- Community education and outreach
- Referrals to other preventive health services
Clients can choose to be seen in Carrington or Jamestown. Reproductive Health Services are available in Carrington the third Tuesday of each month from 8:00 AM – 12:00 PM. Please call 701-252-8130 to schedule an appointment with Central Valley Health District.

Women's Way
Women's Way is a program that pays for breast and cervical screening services for women who meet certain age and income qualifications. Women can have insurance, but that insurance may not cover preventive screening services, or they may have a high deductible.
Central Valley Health District serves Barnes, Foster, LaMoure, Logan, McIntosh, and Stutsman counties.
